The Women's Health Initiative
The Women's Health Initiative (WHI) was released in 2002 and changed how women's hormones were managed during menopause claiming that HRT increased a woman's risk for heart attacks, blood clots and stroke. Approximately 50% of women stopped their hormones overnight and providers stopped prescribing, but what did it really show?
​
Study Background. The women who participated in the study were randomized into two groups depending on their uterine status. Women with intact uterus' were given conjugated equine estrogens (CEE) and a progestin, those without uterus' were started on CEE only.
​
Results. 30 out of 10,000 women in the combination therapy group developed heart attacks, blood clots or strokes, an occurrence rate of .3%, meaning 9,970 women had no events. The combination therapy group was stopped after two years while the CEE only group continued for several more years.
​
Limitations. CEE and progestins are both non bioidentical hormones. The women who developed events were over 63 and had never had any menopausal symptoms. Many were smokers and many had already developed diabetes or heart disease and only developed events in the first year of therapy.
​
Interpretation. Hormones are safe when started before the age of 63, even when using non bioidentical hormones. The risk for women over 63 disappears after 1 year.
