In spite of its name and popular understanding, PCOS is not primarily an ovarian or sex hormone issue. At its root, it is a metabolic issue. Multiple factors including genetics, diet, and lifestyle influence how PCOS presents.​

Visceral fat or "brown fat" accumulates around our internal organs and release chemicals called cytokines which increase insulin resistance and systemic inflammation. Insulin resistance decreases production of a protein called Sex Hormone Binding Globulin (SHBG) whose primary responsibility is to bind up free circulating sex hormones such as estradiol and testosterone. Increased levels of these hormones cause the symptoms commonly associated with PCOS like menstrual irregularities, infertility, acne, and hirsutism.
